Geropsychiatric services provided:

  • Psychiatric and medication evaluation, monitoring and treatment
  • Medical evaluation, monitoring and treatment
  • Multidisciplinary treatment planning and case management
  • Staff trained in Crisis Prevention (CPI)
  • Individual, group and family interventions with a licensed clinical social worker
  • Relapse prevention/recovery-based practices
  • Education and treatment of co-occurring substance abuse
  • Exercise, nutrition and wellness education
  • Leisure and recreational activities

 We offer specialized care for patients with conditions that include:

  • Anxiety
  • Psychosis
  • Schizophrenia
  • Depression
  • Alzheimer’s and Dementia
  • Mental disorders related to brain injury, stroke or other neuropsychiatric disorders
  • Behavior Disturbances

Referral Guidelines:

  • Persons with a significant change in health and functional status resulting from a medical problem(s) and/or hospitalization
  • Persons whose disorders are assessed as being primarily due to mental illness
  • Persons with cognitive, emotional and behavioral problems that require care and treatment in a secure skilled nursing setting
  • Persons who are younger than 62 years of age may be considered for this unit based on their needs
