Geropsychiatric services provided:
- Psychiatric and medication evaluation, monitoring and treatment
- Medical evaluation, monitoring and treatment
- Multidisciplinary treatment planning and case management
- Staff trained in Crisis Prevention (CPI)
- Individual, group and family interventions with a licensed clinical social worker
- Relapse prevention/recovery-based practices
- Education and treatment of co-occurring substance abuse
- Exercise, nutrition and wellness education
- Leisure and recreational activities
We offer specialized care for patients with conditions that include:
- Anxiety
- Psychosis
- Schizophrenia
- Depression
- Alzheimer’s and Dementia
- Mental disorders related to brain injury, stroke or other neuropsychiatric disorders
- Behavior Disturbances
Referral Guidelines:
- Persons with a significant change in health and functional status resulting from a medical problem(s) and/or hospitalization
- Persons whose disorders are assessed as being primarily due to mental illness
- Persons with cognitive, emotional and behavioral problems that require care and treatment in a secure skilled nursing setting
- Persons who are younger than 62 years of age may be considered for this unit based on their needs
