Job Description

Psychiatric Rehabilitation Services Coordinator PRSC Job summary:

  • Provide resident with stable therapeutic relationship
  • Orient the resident to the facility
  • Review and assist the resident in understanding the treatment plan and program schedule
  • Prepare and assist the resident with active participation In the treatment plan review
  • Provide and/or coordinate the delivery of the psychiatric rehabilitation services programs
  • Monitor the resident in the areas of self-directed care and for overall compliance with the treatment plan
  • Completing assigned sections of the MDS, assigned CNA's, and additional components for the resident comprehensive assessment, including: psychosocial, history, skills assessment, discharge planning etc.
  • Re-evaluating the resident on a quarterly basis and documentation the re-evaluation
  • Leading groups and individual counseling sessions as assigned
  • Available to work flexible hours (evening, weekend, holidays)
  • Other case management duties as assigned

Psychiatric Rehabilitation Services Coordinator PRSC qualifications:

  • Bachelors or Masters degree in human service profession (i.e. social work, psychology, sociology, counseling psychology)
  • Demonstrated sensitivity, ability and skills in working with and understanding the needs of the residents, family members, staff members and personnel from community agencies
  • Demonstrated familiarity and knowledge of currents trends in the rehabilitation of the mentally ill
  • The ability to present concise, meaningful written reports and articulate the social/psychological needs of the residents
  • The ability to accept and utilize professional supervision, consultation and in-service training
  • The ability to handle confidential data with professional discretion
  • Experience conducting groups
  • Familiarity with paperwork pertaining to job (ie. MDS, care plans, progress notes)
