Reducing Preventable Rehospitalizations


All BRIA locations utilize the INTERACT (Interventions to Reduce Acute Care Transfers) program to improve care and reduce potentially preventable transfers to the acute hospital. INTERACT is a quality improvement program that focuses on the management of acute change in resident condition. BRIA locations actively engage in Quality Assurance Performance Improvement (QAPI) as part of its collaborative effort in working closely with the IL Quality Improvement Organization (QIO) and meeting the standards in compliance with Federal/State regulations.

The INTERACT program is designed to improve the early identification, evaluation, management, documentation, and communication about acute changes in condition of residents. The INTERACT tools and strategies are already integrated into our everyday care.

Improving communication and collaboration with hospitals is of paramount importance to ensure a smooth and comfortable transition from the hospital. We provide a list of our capabilities to emergency room staff, hospital discharge planners, and hospital physicians so that they understand our capabilities when making discharge decisions. We utilize an acute care transfer list and form to help clearly and succinctly communicate the information that is critical when a resident is transferring to or from a hospital.
